In Arizona, a referral or prescription is not required to see our physical therapists. However, many insurance plans do not pay without referrals from a physician, nurse practitioner, or physician’s assistant. If you prefer to pay without billing insurance, please contact us at 520-572-6540 to discuss private pay options.

In order to schedule an appointment, please contact us at 520-572-6540. Our staff will be able to verify your insurance benefits. In order to promptly and fully answer any questions, please have your insurance card available with the carrier name, ID and group number, and the date of birth of the patient-to-be. We may also request the phone number of the insurance company.

Before starting your therapy, you must have an appointment to be evaluated. Our hours of operation are Monday – Friday, 8:00 am to 5:00 pm. Appointments tend to fill up quickly in the early morning and late afternoon, so call as soon as possible to set up a time to see a therapist. Physical therapy sessions will be at least 45 minutes.
